Campus Proof of Vaccination or Testing
ECC has secured the services of Cleared4Work to collect proof of testing and vaccination. You received a link via text and email. Follow the instructions on the screen to upload an image/photo of your proof and enter additional information about your vaccination. If you are not vaccinated, ECC offers a health professional administered PCR, noninvasive nasal swab, or saliva-based test here on campus. Once you have your test results, you will need to upload them into the Cleared4 system.

ECC will accept proof of testing from other outlets such as Walgreens, CVS, Rapid Test, etc. The Illinois Department of Public Health (IDPH) has given requirements for institutions to accept those tests. It will not be accepted if the test doesn't meet the IDPH standard. Similarly, ECC can accept at-home tests as long as they meet the IDPH standard. Costs of a 3rd party test or at-home/self-test will be the student's responsibility.
All test submissions need to include the following information:
- Student's first and last name
- Date
- Test result (positive or negative)
- Type of test (indicating it is a NAAT or antigen test)
- The entity issuing the result (e.g., laboratory, healthcare entity, or telehealth service)
- Specimen collected date
Know that
- You can upload vaccine proof or connect your electronic health record to your profiles.
- We will ensure CDC vaccination cards are valid and not fraudulent.
- When you have a valid COVID-19 vaccine on file, you will be issued a special BluePass.
- All data is stored in HIPAA-compliant servers.
- We will continue to follow ECC's Case on Campus Contact Tracing Procedure.
If You Test Positive
Student Procedure
Students are directed to self-report confirmed positive/exposure COVID cases to their instructor or appropriate employee.
The instructor or another employee contacts their Dean immediately and provides the following information:
- Name of Student
- Students ID number
- Student course and section number or service area affected
The Dean contacts the COVID Response Team by email at StudentCRT@elgin.edu.
The COVID Coordinator emails the student requesting additional information and copies the other COVID Response Team members.
- Location and length of exposure (i.e., home, public outing, etc.)
- Date symptoms started
- Date of testing (for confirmed case)
- Date of confirmed positive results (for confirmed case)
- The last date the student was on campus
- The areas the student visited while on campus, including locations 48 hours before symptoms started or receiving a positive COVID test result.
- Identify if the student is enrolled in other classes and/or sections.
The team will then identify if the student is enrolled in other classes and/or sections.
Employee Procedure
Employees are directed to self-report confirmed positive/exposure COVID cases to their supervisor or another appropriate employee. The supervisor or another employee contacts Human Resources immediately and provides the following information:
- Name of Employee
- Employee ID number
- Service area and Employee Department affected
Human Resources contacts the COVID Response Team by email at StaffLiaison@elgin.edu.
The COVID Coordinator (a member of the COVID Response team) emails requesting additional information from the employee and copies the other COVID Response Team members.
- Location and length of exposure (i.e., home, public outing, etc.)
- Date symptoms started
- Date of testing (for confirmed case)
- Date of confirmed positive results (for confirmed case)
- The last date the employee was on campus
- Areas the employee visited while on campus, including locations 48 hours before symptoms started or receiving a positive COVID test result.
